Several are available on the W.W.W. now but are difficult to find. It appears that these html/css services are taking advantage of good marketing of what's already available, open source.
Less than 1% of the billions of web pages indexed on the W.W.W. are valid. Any service that increases this to at least 2% is worthwhile.

I used XHTMLized for a recent project and they were amazing. They helped out after they had done the first page making some adjustments I needed, and they tested it in IE7 (this was before IE7 was getting popular).

I suggest that somebody open a group action on fundable to collect fund to benchmark the quality of the xhtml/css produced by all these services. Then one unique design would be sent to all of them and then the code quality would be compared along with other things such as delay, browser supported,...
I had some work done by SliceMyDesign.com and they were great and very timely. I highly recommend them if you need something quick and would like to work with a down to earth people.

When I have too much coding to do ourselves, I've used a couple of services on this list. The best I've sampled so far has been psd2html.com - they're fast, reliable, accommodating, and their code is excellent. I've used their cheapest package once and their most expensive (which is relative - it's only $211, I think) twice, and I think it's worth paying the extra. Can't fault them at all.
Notice that PSDXHTML.COM offers tableless xhtml strict cross-browser compatibility including Safari support at only $89 ! This is the lowest price available for such xhtml quality!

Do not use DesignSlicer, the owner feels that his company is a "test" and thus he doesn't have to take his work seriously, customer service sucked and I'm overall not happy with their service.

I have tried a couple of companies listed here. But I am using http://www.psd2website.com for my XHTML/CSS projects. They provide a quality work and coding standard are better than any other services.
just for your reference - I contacted both XHTML Slicer and PSD2HTML and the first said they are too busy and the other has yet to reply after 2 business days. The time frames stated on their sites are not accurate if you're looking for fast turnaround.
I first tested the service offered by PSD2HTML.com with my own web site to make sure that I would be confident using their services for my clients. I run a small web design and development company in the UK and creating good markup and CSS is a time consuming but important job for us. The results we received back for our web site were perfect. There was absolutely no difference when looking at the source file and the html side by side, and it wasn't a simple layout either. We have since used them on 2 further jobs - again, perfect, and as I type, I know I have another 2 to send today with more coming up. As far as I'm concerned, there is no better way to do it. Even on simple jobs it will work out much more cost effective to outsource this than to do it in-house.
Our firm is always trying to push the boundaries of design. Even with our complicated templates, PSD2HTML.com have no problem creating validated and clean code. No corners were cut here. It's quite a convenience to have such a company at our finger tips where we can turn to have the same quality work accomplished that we would do with our in-house staff. PSD2HTML.com met their promised deadline and maintained great communication. No problems here!

As an update to previous thoughts from me here - I still have no problem with psd2html.com's service, but I've been using xhtmlized.com more and more and am extremely happy with their service. It's faultless - as is their code.
Hi from Spain. My clients (Spain's #1 online newspaper) are really picky, and we worked on their site's redesign for months, changing layers and fonts and effects over and over, so at the end of the process we had a couple of incredibly messy PSD files, with over 400 layers each. I sent them to PSD2HTML, thinking that no human being could ever convert these files into something that could be usable. Their people came back asking very few questions and all of the sudden, I had an astonishing markup, really easy to understand, clean and 100% compatible. It was a pleasure to work with them. From now on, I'll design my websites only with Photoshop.

Might as well throw my hat in to the ring! I just launched my design to xhtml service. Prices vary depending on design complexity, I'm always flexible about build or design tweaks. All builds are cross browser tested, I build email templates too if anyone's interested.
Wow! This post was made in November of last year and its still alive- so here goes- One thing I see sorely missing in this market is coders who understand design- I feel its critical to catch the nuances- that 2px offset is just not on for a designer- The coder just might not even see it- I think it stems from an old-fashioned attribute- respect. There's not much going around these days so me and some friends set up this high-quality xhtml service and called it the old-fashioned way of doing things- Retro XHTML. In fact I wanted to advertise in this site but Jonathan was all booked- But in the mean time I've had a few good clients who seem to appreciate my approach to this (God bless them!). Does anyone even agree?
